I regret not doing a lot of things in life like telling that bully in the third grade that I was not afraid of him or telling a teacher that I really appreciated all she had done for me.   The one regret that I have that has really changed where I am in my life when I messed up an opportunity that I was given to be a â€Å"third key† manager at a high end retail establishment shortly after graduating from high school. I did not get the position because I failed a urine screening for drugs.   I had smoked a little pot (three puffs to be exact) a few days prior to my interview in â€Å"celebration† of this opportunity as my friends and I hung out and partied.   I had no idea at the time that a little pleasurable puff of paradise (39) from this marijuana cigarette would end the most promising career opportunity that I may ever have had.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   After gradating from high school, I was on top of the world and loving life.   My life was like a box of chocolates (25), full of delicious and hidden opportunities.   I landed an interview for an amiable administrator’s (57) position and everything went well during the interview.   I was offered the job upon condition of a passed drug urine screening.   Drug urine screening?(12).   Say what?!   I had never had one of those before, probably because I had never had a job worthy enough of expelling urine for someone to analyze.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Not only was I highly unprepared for this condition of my employment, but I was actually surprisingly very sure that I was going to pass the screening despite the fact that I had smoked some marijuana three days earlier (58).   At the time, I was unaware of how long drug traces remained in my system, so I thought I would be good to go, but deep down, something did not feel quite right.   Another part of me told me that I was going to lose this once in a lifetime opportunity (55) due to a stupid choice that I made one night to party and get high with my friend Andy (69).   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Why didn’t I just say no, as all the commercials and billboards had been urging me to do for years (24)?   There is only one logical reason, and that is because I was stupid.   No, that is not a logical reason; that is a lame excuse.   I did it because I was selfish (17).   I was more worried about my image at the time than my own future (19).   Selfish†¦selfish†¦selfish (47).   And stupid.   Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚  Ã‚   Losing the job that I had wanted so badly and that could have made a real difference in my life was highly disturbing.   I disappointed myself in the worst way.   I ended up working several mediocre jobs after that.   However, as the old saying goes, when life gives you lemons, make lemonade (51).   So, I made lemonade all the way to college.   Here I can advance my pool of job opportunities (20) and with the pool of knowledge that I already posses from previous mistakes, I will be able to do more than make lemonade.   I can make a better career for myself and can become a better person.

Book Banning Essay

Each year, libraries across the United States report hundreds of challenges. The leading causes for contesting a book are sexually explicit content, offensive language and inappropriate subjects for minors [source: American Library Association]. Only a minority of the requests actually make it through to banning the book from its respective library. The Catcher in the Rye. The Scarlet Letter. Huckleberry Finn. Harry Potter. The Diary of Anne Frank. Animal Farm. To Kill a Mockingbird. The Da Vinci Code. The Grapes of Wrath. These literary classics have been vital to the education of many, especially children and adolescents. These great novels both teach important values and educate children about world affairs and classic themes. Unfortunately, each of these novels has been banned at one point in time. Many of these classic stories have been banned because of sexual references, racial slurs, religious intolerance, or supposed witchcraft promotion. Although some may consider these books controversial or inappropriate, many English classes have required us to read these books. Like the teachers that assigned us these books, I believe that even controversial books can ultimately boost, not deter, our educational wealth. I oppose book banning for three main reasons. First, I believe that education should be open to everyone. Everyone should have an opportunity to read any literature of their choosing and form his or her own opinions based on the reading. Micah Issitt lists â€Å"three basic rights covered under the freedom of the press: the right to publish, the right to confidentiality of sources, and the right of citizens to access the products of the press. † My second reason specifically addresses the last right stating that citizens should have access to the press. The government should not restrict books from being published or interfere into personal affairs as this is an infringement of the First Amendment. Finally, I believe that parents should monitor what their own children read, but not have the authority to ban other children from reading these novels. For these reasons, I conclude that the government should play no role in the issue what citizens do and do not read, and that book restriction should remain a solely private matter. At first glance, the debate over banning books appears unimportant. Nevertheless, this debate has divided our nation into those who favor censoring books to protect their impressionable adolescents, and those who argue that education should be open for everybody without interference from the government in restricting the publishing and accessing of these books. Issitt argues that censoring books violates the First Amendment, stating that â€Å"citizens must be free to seek out any media, regardless of content, that they deem appropriate for entertainment, information, or education. Denying the rights of the consumer, in any area, is one of the hallmarks of authoritarianism. † While I do not equate banning books with â€Å"authoritarianism,† we do endorse Issitt’s belief that individual citizens have the right to choose, under their own discretion, what books to read. The First Amendment protects the freedom of expression and speech, and by prohibiting certain messages, the government clearly infringes upon public rights. On the other hand, Healey claims that censorship does not â€Å"repress information that teenagers and children are exposed to,† but merely gives parents the rights to educate their children in the ways they deem appropriate. Though I concede that parents do have the right to monitor what their children read, they do not have the right to remove books from public libraries or monitor what other children in the city read. Healey attempts to persuade readers that â€Å"censorship of books should not be about silencing voices on important topics, but about steering young people toward the best possible literature;† however, she fails to specify what constitutes as â€Å"the best possible literature. † Some of â€Å"the best possible literatures† also happen to cause the most controversy, including Huck Finn, Harry Potter, The Scarlet Letter, and To Kill a Mockingbird. Those who protest against these books have clearly not studied them in depth. For example, the main theme in Huckleberry Finn focuses not on advocating racism, as some suggest, but proving that race does not define a person’s intelligence or capability for compassion. Even Healey admits that â€Å"concerned parents and community members react without taking the time to closely investigate the books they want banned. † While I agree that parents should play an active role in educating their children and as their primary guardians, have the legal right to monitor what their children read, I disagree that this legal right extends to controlling what other children in the neighborhood read as well. Prohibiting children from reading a book will not enhance their moral values. Rather, banning a book more likely will increase curiosity for reading it. I also empathize with parents who ban books with controversial or uncomfortable subjects because they are unsure as to how their children will react or how to explain such topics. A good way to discuss these subjects with children is to read books with various views on the subject so that children can experience multiple points of view before forming their own opinions. Healey herself agrees that such a method â€Å"might help young people better understand the world they live in, the human condition, and issues they face in their culture. † As Healey stated, parents also tend to ban books based on â€Å"moral grounds, although some books have been condemned for their perspectives on civic values and history. † For this very reason, the general public should read these books. Our society, especially our younger children, needs to read these books since fully understanding a topic requires knowledge of both sides. If we choose to disregard even a highly unpopular opinion, we intentionally choose to live in ignorance, only partially educated in a topic we claim to know so well. Without a doubt, if we continue to ban books and ignore what some consider taboo topics, we hinder ourselves and our children from finding ways to solve society’s problems, thus hampering the development of our nation as a whole. Many conservative groups make the argument that the books that have been banned have material that is inappropriate, immoral or contradicting the beliefs they have ingrained in their children and/or their society. Take for consideration the controversial books that tackle difficult, touchy social issues like homosexuality. Books like â€Å"Heather Has Two Mommies,† by Leslea Newman and â€Å"Daddy’s Roommate† by Michael Willhoite (both books written for youth with gay parents) were shot down by conservative groups because they attempted to educate children about homosexuality, an issue parents felt needed to be taught to their respective children by them. While this may seem like a valid argument, really it is just skirting around the actual issue. Book-banning cases usually concern the protection of children and their innocence, but all that is happening is sheltering parents showing an awkward avoidance of their children’s confrontation with uncomfortable matters. It is not only selfish, but also harmful to the overall education of their children. This act of prohibiting books is just the parents’ way of evading of the conversation with their child about these sensitive issues. These two books are issues that Healey brings up in her argument on how groups were upset about the way these books informed their children of homosexuality. Homosexuality and other touchy social issues are part of everyday life, and for a group to attempt to censor this subject from younger society is almost absurd; these issues are not monstrous and the censorship of them not only shows prejudice but lack of respect. Banning books seems to be the most public solution for a private matter- not everyone should have to suffer restrictions because one group feels uncomfortable with the book. That being said, there are often books that contain graphic and often highly inappropriate material; I do consent that these books should be censored at the discretion of the parent, or anyone involved however, no one is forcing books upon others, so we should not be forced to remove them. Other groups would say that it’s also the duty of the government to regulate these books to protect concerned citizens and their families, but I would have to disagree. It’s the exact opposite of the government’s role- our private lives, the books we read, should be regulated and controlled by us. Banning books from public congregations is not what the government was intended to do. Topics that seem socially outlawed in public, let alone published, have been banned because their immoral content may have a negative effect on younger children. In these books, authors doesn’t promote or encourage bad behaviors, they prepare their readers for some of the real world challenges. The child would never be able to learn these things if the book was banned, nor be able to form his or her own opinion about that certain topic. Healey discusses that the book, 33 Snowfish, a â€Å"dark story of three teenage runaways who are victims of various forms of abuse†¦ † by Adam Rapp may be an unsuitable way to educate children on these timely topics. However, having these stories banned all together would just further shelter a child whose parents may not be willing to discuss these issues with them at all. Even though these books center around scary topics, they are educating children on real life matters that they will be exposed to once they venture into the world themselves. Healey goes on to make the point that the books should not be banned as well, since it is a matter of private opinion not one to be made by the public libraries of a community. She suggests that schools should â€Å"inform parents about the kinds of books they offer children† in their libraries and classrooms instead of banning them. With the knowledge that some of these books have to offer, children can learn how not to act and what can be the consequences if they do misbehave. This learning experience could turn around with the help of a parent and pass a positive affect over the child. Clearly, banning books not only hinders a child’s educational development but also leaves them unaware of the true state of the world. Books do not simply impart general information; they heavily influence a child, the future generation. Without regular access to books, both adults and children could not form sound opinions, only narrow-minded ones. Both advocates and opposers of book banning agree that â€Å"books are powerful instruments. † Otherwise, a debate on the subject would neither have arisen nor lasted so long. Because books â€Å"can be used to†¦ inculcate values and transmit ideology, and to stimulate the imagination,† as Healey suggests, any person should remain free to select his or her reading material. This personal issue of selecting reading material has no relation to the government. On the contrary, government action interferes with individual education, a primary American value. Ultimately, children can learn personal responsibility in determining which books to regard and which to discard. In the future, these children will become well-educated adults who can benefit the American society. ————————————————-

There are three different types of communication in the medical field: traditional communication, electronic communication, communication through social media. There are advantages and disadvantages to each different type of communication. Traditional communication includes speaking face to face and mail (letters). Electronic modes of communication are email phone (home or cell phone), texting, and chatting. Social media involves communicating with several people or just one person at a time using public forums or private messaging through such websites facebook and twitter. Each and every type of communication is equally as important as the other but they all have their own advantages and disadvantages. Traditional forms of communication as simple as can be but they can confuse anyone if every step is not done correctly. You would think that speaking to someone face to face would be the best way to communicate but this is not always the case. Anyone can miscommunicate what they are trying to say. There can be other barriers like language barriers and also emotional barriers. If someone speaks a different language it can be hard to understand them unless there is an interpreter available, which isn’t always possible. Emotional barriers are hard because when health care is involved things can go wrong and people then become angry, sad, and/or tired. When this happens they may not want to listen to or respond to their doctors. Face to face communication is good too because you can ask questions then and there, and receive advice from your physician without having to wait too long. Mail can be very annoying because it can take days or weeks to reach its destination and more days or weeks to receive a response. The only good thing about mail would be that you will have documentation of your physician’s opinions and test results. Traditional communication methods have been around for a long time but times are changing and these types of communication are becoming few and far between. Electronic communication seems to be where most people end up communication, even to our doctors and other health care officials. We call, email, and even text to tell doctors and nurses our health care problems before we even consider actually going to a doctor’s office and speaking to the doctor in person. These communication techniques are good because we can receive answers to our questions also instantly if the health care official isn’t busy. One disadvantage is that someone may not get back to you as soon as you would like; which will leave you at home, wondering what you should do. Electronic communication isn’t exactly the best form of health care because if the doctor cannot actually see the patient and test their symptoms they cannot defiantly tell a patient what is wrong with them. Social media is becoming more and more popular every day because it is a way to keep in touch with people we don’t see every day. It has also become home for all kinds of new and old information, including health care information. This can be a great way to communicate about health care because people can learn new things about health care and medicine every day. People can also communicate with organizations full of doctors and nurses who can teach patients about any news in the health care world and advise patients on how to take care of themselves. This may sound great but there are terrible disadvantages to this type of health care communication. You never know who you are actually speaking to on the internet. What could appear to be an organization of doctors and nurses who want to offer free health care advice could be someone who knows absolutely nothing about medicine. Social media websites are not the best place to look for any sort of medical advice because patients want real help.

Truly, organizational culture may be perceived as the manner in which an organization solves problems to achieve its specific goals and to maintain itself over time, it is holistic, historically determined, socially constructed and difficult to change (Hofstede, 1980). The reason that this appealed to Zimbardo was he was a former class mate of Milgram and was interested in expanding on his research. Zimbardo set the experiment by first placing an advert in the paper asking for male students to participant in a prison life experiment. The students who applied there were 24 in total they were selected from larger group that had applied for the study because they had no psychological issues, medical condition and no criminal backgrounds. They were all agreed to be in for one to two weeks for the experiment. The experiment was due to last two weeks but this had to be stopped early due to what was happening to the participants. The guards became abusive and prisoners began to show signs of extreme anxiety and stress. Neither the prisoners nor the guards were advised on how to interact with the each other. The guards began to behave like real life guards but they were also aggressive towards the prisoners, and the prisoners became passive and depressed. Five of the prisoners began to experience such severe negative emotions, that they had to be released from the study early. According to Zimbardo and his colleagues, experiment shows the powerful role that a situation can play in human behaviour. Because the guards were placed in a position of power, they began to behave in ways they would not normally in their everyday lives or in other situations. The prisoners, placed in a situation where they had no real control, became passive and depressed. Clinical features may range from asymptomatic eosinophilic infiltrates to acute eosinophilic pneumonia (AEP)-like syndrome leading to acute respiratory failure. This report describes the first case of clomipramine-induced AEP-like syndrome causing acute hypoxemic respiratory failure which was treated successfully. In conclusion, considering the increasing use of tricyclic antidepressants, physicians should be aware of this very rare and life-threatening complication of clomipramine. KeyWords: Clomipramine, acute eosinophilic pneumonia-like syndrome, treatment INTRODUCTION Many drugs have been associated with pulmonary complications of various types with an increasing number of the therapeutics. Respiratory system is a target for a variety of drugs because of its large contact surface and acting as a metabolism site for drugs. The patterns observed in drug-induced pulmonary toxicity are highly variable and depend on the localization of adverse reaction. Since most of the drug-induced pulmonary toxicities involve the lung parenchyma, the most common form of drug-induced lung toxicity is interstitial lung disease (1). Drugs theoretically can produce all histopathological patterns of interstitial lung disease, including hypersensitivity pneumonitis, organizing pneumonia, bronchiolitis obliterans organizing pneumonia, acute respiratory distress syndrome, granulomatous pneumonitis, and eosinophilic lung disease (2). More than 350 drugs are known to cause acute eosinophilic pneumonia (AEP)-like syndrome (3, 4). However pulmonary complications related to tricyclic antidepressants are uncommon. To our knowledge, only one case of AEP associated with the usage of clomipramine has been reported in PubMed in 1999, and in the aforementioned case clomipramine had been used not alone but with sertraline (5). This report describes the first and successfully treated severe case of AEP-like syndrome associated with clomipramine treatment alone. Written informed consent of the participant patient was obtained for the publication of this case report and any accompanying images. CASE REPORT A 38-year-old female was referred to our clinic from psychiatry department of another hospital. She had been newly diagnosed with panic attack and anxiety disorder one week prior to the appearance of respiratory symptoms. After admission to psychiatry polyclinic, clomipramine therapy was prescribed at a dose of 75 mg/day. After 6 days of clomipramine therapy, the patient exhibited dyspnea, increasing shortness of breath, dry cough, chest pain, and fever up to 39.20C. She was a housewife and had no work-life history. There was no past history of a chronic disease and her smoking history was 16 packs/year. She had never consumed illicit drugs, had no allergies, and had not visited a foreign country during the previous 5 years. On admission, the patient was in a respiratory distress, with a respiratory rate of 35 breaths/minute, a blood pressure of 110/70 mmHg, and a heart rate of about 140 beats/minute with sinus tachycardia. Examination of blood samples revealed only anemia (haemoglobin:9.4 g/dl). Number of leukocytes, thrombocytes, and results of blood chemistry were within normal limits. Erythrocyte sedimentation rate was 40 mm/h and serum level of C-reactive protein was 4.5 mg/dl. Measurement of arterial blood gas analysis on room air revealed pH: 7.44, PaCO2: 32 mmHg, PaO2: 49 mmHg, HCO3: 27 mmol/L and SaO2: 86% which was compatible with hypoxemic respiratory failure. Laboratory results including HIV testing, serological screening for vasculitis [autoantibodies to deoxyribonucleic acid (DNA), double stranded DNA, proteinase-3, myeloperoxidase, rheumatoid factor, and circulating immune complexes] and various pulmonary pathogens were negative. Stool tests for ova and pulmonary parasites were normal. The level of total serum immunoglobulin E (Ig E) was within normal limits (65 IU/ml) and antigen-spesific Ig E against Aspergillus fumigatus and Candida was negative. Her chest radiograph revealed a homogenous dense shadow located especially in the lower field of the right lung and some additional reticular opasities in the lower zone of the left lung (Figure 1). Computerized Tomography (CT) (Brilliance; Philips Medical System, Eindhoven, The Netherlands) of the thorax demonstrated patchy areas of ground-glass, thickened interlobular septae and homogenous consolidations with air-bronchograms especially in the middle and lower zones of the right lung (Figure 2). The patient was admitted to the intensive care unit because of the need for non-invasive mechanical ventilation. She underwent bronchoscopy (Aquilion; Toshiba, Tokyo, Japan) with bronchoalveolar lavage (BAL). During the bronchoscopy procedure the bronchi appeared inflamed and contained increased secretions. Gram stain and cultures of the bronchial washings did not reveal any infectious organism. Total cell count on BAL was 182 cells/mL with 30% eosinophils, 8% neutrophils, 55% macrophages, and 7% lymphocytes. Special stains and cultures for mycobacteria, Pneumocystis jiroveci, fungi, Legionella pneumophila were negative. Serology testing for cold agglutinins were negative. Intravenous methylprednisolone 60 mg/day (1 mg/kg/day) was initiated on admission. Then dose of the methylprednisolone was gradually tapered and methylprednisolone was discontinued after 4 weeks. The chest radiograph of the patient cleared markedly within 15 days of the treatment (Figure 3). The patient was discharged 18 days after admission. Follow-up examinations one and four weeks after discharge revealed normal laboratory results and a normal chest radiograph. Pulmonary function tests 3 months after admission showed no ventilatory defect. Last contact with her was a polyclinic visit for control one year after discharge and she was free of respiratory symptoms. DISCUSSION The eosinophilic lung diseases are a heterogeneous group of pulmonary disorders characterized by increased numbers of eosinophils in the airway or lung parenchyma, and drugs are well known causes of eosinophilic lung disease (6). Patients with drug-induced eosinophilic lung disease can have variable presentations, ranging from asymptomatic transient eosinophilic infiltrates to AEP-like syndrome leading to acute respiratory failure, depending on the drug involved. AEP is described as a clinical entity which involves acute onset of symptoms within ≠¤5-7 days, fever ≠¥37.20C, hypoxemia, bilateral alveolar or mixed alveolar and interstitial infiltrates, and lung eosinophilia (percentage of eosinophils ≠¥25% in BAL and/or predominance of eosinophils in open lung biopsy). In addition, without any history of hypersensitivity to drugs, no evidence of infection, and no other known cause of eosinophilic lung disease (7, 8). Similarly, patient in this case had newly onset respiratory symptoms, fever and mixed alveolar and interstitial infiltrates suggesting an interstitial lung disease. In addition, a bronchoscopy with BAL revealed presence of importantly increased percentage of eosinophils (30%) in the lavage fluid. Based on the results of BAL, radiological features and her clinical course, she was diagnosed as potentially having an AEP-like syndrome related to clomipramine. Unlike patients with other eosinophilic lung diseases, most of the patients with AEP have normal peripheral blood eosinophil count (8). The level of total serum IgE can be elevated, however it is not used as a diagnostic tool (9). In this case, the patient had both normal peripheral blood eosinophil count and normal level of total serum Ig E. The diagnosis can easily be obtained with BAL and lung biopsy is not an obligation for the diagnosis of AEP in most cases. If needed, the purpose of lung biopsy is to exclude other diseases that can mimic AEP. When lung biopsy is performed, the histopathological features are characterized by diffuse alveolar damage with interstitial and alveolar eosinophils (10). There was no need for lung biopsy in this case and diagnosis could be established with the help of BAL cell count. In some cases discontinuation of the drug is enough for resolution of the radiological features. However, some patients experience a severe disease and require corticosteroid treatment. Generally, response to corticosteroids is rapid and most of the patients have significant clinical improvement within 24 to 48 hours. The optimal dose and duration of corticosteroids have not been determined exactly (8, 10). Commonly used doses of intravenous methylprednisolone in case series range from 60 to 1000 mg per day until respiratory failure resolves. Thereafter, the patient can be switched to oral prednisone, and the steroids can be tapered off over 2 to 12 weeks. In this case, 60 mg/day intravenous methylprednisolone was administered on admission and dose of the corticosteroid was regulated according to resolution of radiological findings. Corticosteroid treatment was terminated after 4 weeks. In conclusion, physicians should be aware of rare, life-threatening and treatable AEP-like syndrome because of clomipramine.
